Using the online Zanda booking service for Conscious Mind Centre, you agree to the following:
1. I have read and agree to Conscious Mind Centre's fees.
* $230.00 for standard 50-minute appointment.
* $345.00 for extended 80-minute appointment.
2. I have read and agree to Conscious Mind Centre's 48-hour cancellation policy.
3. I understand and accept Conscious Mind Centre's cancellation fees for late cancellation and no-show appointments.
* $180.00 fee for less than 48-hours notice.
* Full appointment fee for less than 24-hours notice.
4. I have read and agree to Conscious Mind Centre's privacy policy.
Conscious Mind Centre's cancellation policy and privacy policy can be found on our website. Alternatively, you can make a booking with our administration team on 07 5616 2119.
TELEHEALTH APPOINTMENT NOTE.
All telehealth appointments require payment in full prior to your appointment time. On completion of you appointment any applicable Medicare rebates will be processed automatically.
Our administration will contact you on the day of your appointment for payment. All telehealth links will be sent on the day of your telehealth appointment.
