Welcome to the TPC portal. Please register using the Log in button above. Use My Forms below (once registered), or in your welcome email click the link 'New Client Intake Form' to fill out our referral form. YOU MUST click the FINISH button at the bottom of the form or we won't receive any information. You will not be added to the waiting list if you do not complete the form properly and click FINISH.
Please note that referring yourself means you will be a private, fee-paying client (this also applies to GP referrals). Currently our hourly charge is $184 (incl GST). Also, please be aware that our waiting list is quite lengthy, currently around 9-10 months.
For children, please fill out the form with their details, as they are the client.
Please check for emails from us (including your spam/junk folder) as we may require or have information that is necessary to proceed with your referral.
Your completed form will be kept in the My Forms area below. After you have been placed on the waiting list you will be contacted in due course - it may be several months due to our lengthy waiting list.
When you become an active client you may view your appointments, and cancel if needed. For anything else please call 07 846 6907 or email tpc@tpc.org.nz
