LGMH is currently accepting limited new client referrals for Ballarat based and Telehealth clients.
If you are not an open and active client with LGMH [meaning you have attended an appointment within the last 3 months] you must submit a self-referral form via the website [www.lgmh.au] and await contact from LGMH before you proceed to book appointments via this online portal.
CURRENT LGMH CLIENTS:
Please take note of the following before booking:
Unless LGMH has advised you of a fee increase, your usual individual fee still applies and is to be paid on the day of your appointment.
When you make an appointment, that time is set aside specifically for you. If you do not attend your appointment or cancel at short notice, that time cannot be offered to others who may be waiting for support.
Please take note of updated booking terms and conditions:
- 48 hours notice is required for cancellation or rescheduling of your appointment.
- To cancel or reschedule your appointment, please contact LGMH via text or email (please don't do this via the portal).
- If 48 hours notice is not provided for cancellation or rescheduling of your appointment, the full amount of your appointment will apply.
