LGMH is currently accepting limited new client referrals for Ballarat based and Telehealth clients.

If you are not an open and active client with LGMH [this includes previously engaged clients who have since had their care closed] you must submit a self-referral form via the website [www.lgmh.au] and await contact from LGMH before you proceed to book appointments via this online portal.

Please note that LGMH operates on a 'sliding scale' fee structure, meaning that the fee quoted on this portal may not apply to you depending on your circumstances. This can be discussed further with LGMH upon self-referral.

Please take note of the following before booking:

Your individual fee payable may vary from what is indicated as the total fee on the client portal.

Unless LGMH has advised you of a fee increase, your usual individual fee still applies.

When you make an appointment, that time is set aside specifically for you. If you do not attend your appointment or cancel at short notice, that time cannot be offered to others who may be waiting for support.

Please take note of updated booking terms and conditions:

1. Booking an appointment time via this portal requires a deposit of $50.00 to confirm your date and time.
2. The balance of your fee is to be paid on the day of your appointment via EFTPOS or direct bank transfer.
3. 48 hours notice is required for cancellation or rescheduling of your appointment.
4. To cancel or reschedule your appointment, please contact LGMH via text or email (please don't do this via the portal).
5. If 48 hours notice is not provided for cancellation or rescheduling of your appointment, your deposit will be retained as a cancellation fee.
