Please click on the "Log In " button to log in to our secure portal - The "Log in" button is located on the top right of this page.
If you are a NEW Meaningful Pathways client, please click on the "REGISTER HERE" button.
Click on the "My FIles" button below to upload your
1. Referral, and
2. Mental Health Treatment Plan (if applicable)
Once you have uploaded ALL of your documents, click on the "FINISH" button - The "FINISH" button is located on the bottom right of this page.
Please ensure that
• your documents are clearly scanned or photographed (PDF or image files are usually accepted)
• you include ALL of the pages of your Mental Health Treatment Plan
When you register on the portal, you will be sent an email that contains a link to upload your GP Referral and Mental Health Treatment Plan. This email can be disregarded if you upload the documents as instructed above.
Once a member of our administration team reviews your document/s, we will email you our introductory email.
Our practice uses Zanda to collect and retain client information. Zanda complies with the Privacy Act 1988, Australian Privacy Principles, is aligned with Australian Standards, and is ISO 27001 certified, which is the leading international standard for Information Security Management Systems.
