Paediatric Patients
When registering in the portal, please use the name of the child the appointment will be scheduled for (not the parent). Parent and Carer details can be added at the appointment.
For all Paediatric appointments (18 Years or younger) please bring your Blue Book with you.
If your child is Preschool aged and you have younger siblings who will be attending, we encourage additional caregivers to attend the appointment. Extra hands to support younger siblings will mean the child being assessed will perform their best and reduce the need to have to return to try again another day. So, if you are lucky enough to have a Village, please call on it and maybe stay on for a babycino and smiley face biscuit at Mr Chu's afterwards.
Adult Patients
Adult clients are welcome to bring their frequent communication partners with them. This may be a family member who can attend and provide support during your appointment.
All New Patients
If you are a new patient, an email will be sent containing the link to your new patient intake form. Please complete this as soon as possible. It will need to be submitted prior to your appointment. It should take less than 10 minutes to complete your new patient intake form. You will only be required to complete the new patient intake form once. Please also review the Practice policy and Engagement documents prior to attending your appointment. If you have not received your new patient email, please check your Junk or Spam folder in case it has been directed there by your email filter. Please rescue it and send it to your Inbox so it is not lost!
