Paediatric Patients
When registering a child in the portal, please use the name of the child for whom the appointment will be scheduled (not the parent). Parent and Carer details can be added at the appointment. Don't worry if you get this part wrong. We can fix it during the appointment.
Please schedule a Standard Appointment for children aged 1 year and older.
Please schedule a Long Consultation for children aged younger than 1 year, as two audiologists will be required to support your appointment.
For all Paediatric appointments (18 Years or younger) please bring your Blue Book with you.
Adult Patients
Adult clients are welcome to bring their frequent communication partners with them. This may be a family member who can attend and provide support during your appointment.
All New Patients
If you are a new patient, an email will be sent containing the link to your new patient intake form. Please complete this as soon as possible. It will need to be submitted prior to your appointment. It should take less than 10 minutes to complete your new patient intake form. You will only be required to complete the new patient intake form once. Please also complete the Practice policy and Engagement documents prior to attending your appointment.
If you have not received your new patient email, please check your Junk or Spam folder in case it has been directed there by your email filter. Please rescue it and send it to your Inbox so it is not lost!
