Important Information
✅ Complete Your Forms: Please complete your online intake and consent forms here and access them anytime (excludes DVA/Open Arms and other third-party forms). Forms may be updated periodically in line with APS ethical standards, legal requirements, and practice updates (at least annually or as needed).
✅ EMDR Clients: 90-minute EMDR sessions are for EMDR clients only. If you need a different time, email or SMS us.
✅ Cancellations: Cancel at least 3 days in advance to avoid fees.
How to Cancel an Appointment
