Standard sessions are 50 minutes long and $220.
LOCATIONS: Telehealth appointments are available all week. Face to face appointments are at the following locations:
~ Monday, Tuesday & Wednesday from Form & Practice, Olinda
~ Thursday & Friday from The Hills Medical, Olinda
NEW CLIENTS: A deposit of $110 is required to book your initial consultation. Following booking you will receive an email with intake documentation which we ask you complete prior to your first appointment.
MENTAL HEALTH CARE PLANS: For those seeking assistance under a MHCP & GP referral, a Medicare rebate of $96.65 can only be claimed after full payment has been completed. Claims for rebates are submitted on your behalf if all required information is provided.
~ Please note that referral documentation must be supplied prior to a claim for rebate can be processed.
~ Clients are responsible for claiming their own rebates from private health insurance companies.
